The Puyallup School District Native American Education Program was established in 1973, and is funded by a formula
grant from the Office of Indian Education under the Department of
Education (Title VI of the Elementary and Secondary Education Act) and
the Bureau of Indian Affairs (BIA).
The Native American Education Program currently
serves 364 students in 32 schools representing 626 Native American
tribes, bands, or groups in the Puyallup School District and continues
to grow. The top five tribes represented among Puyallup students are:
• Alaska Native
• Puyallup
• Cherokee
• Choctaw
• Chippewa
To enroll in the Puyallup School District Title VI Native American Education Program:
Student must be enrolled in the Puyallup School District and Parents/guardians must complete a Title
VI Student Eligibility Certification Form (ED 506 form) for
students to receive program benefits and services. For a list of program

requires the student, parent, or grandparent have Tribal membership
(membership/enrollment number or other proof) with a Federally

be completed every year. ED 506 forms will be maintained in the Native American Education Program office and information on the form is confidential and
not shared with others.
Definition: Indian means any individual who is:
• A member (as defined by the Indian Tribe or band) of an Indian Tribe or Band, including those Indian tribes or bands terminated since 1940, and those recognized by the State in which the tribe or band reside; or
• A descendant in the first or second degree (parent or grandparent) as described in (1); or
• An Eskimo or Aleut or other Alaska Native; or
• Member of an organized Indian group that received a grant under the Indian Education Act of 1988 as it was in effect: October 19, 1994.
