KINDERGARTEN REGISTRATION 2023-2024 SCHOOL YEAR HEALTH CARE REQUIREMENTS

Life Threatening Condition Law 

Washington State law requires that children with life threatening conditions have medications and/or 504 treatment plan on file before attending school. A life-threatening condition is defined as one that puts the student in danger of death during the school day. 

• Meet or call the school nurse to develop an individual emergency treatment plan now, or within the first two weeks before school starts.
• The Emergency Action Plan or Individual Health Care Plan, signed and dated by parent and health care provider, must be in place before the first day of school.
• Physician order for medication at school, signed and dated by the health care provider and parent must be in place before the first day of school.
• Medications for life threatening conditions must be in place before the first day of school.

Health History Form must be completed, signed, and dated. Please distinguish if the condition is current or past history.

Certificate of Immunization Form (CIS)

• Any child attending school in Washington State is required by law to be fully immunized according to state guidelines.

• Starting August 1, 2020, all immunization records turned in to schools or childcare centers are required by state law to be medically verified. Immunization records must be turned into the school on or before the first day of attendance. This means immunization records turned in to the school must be from a health care provider, or you must attach paperwork from a health care provider to your handwritten form that shows your child’s records are accurate. Your child cannot attend school until you provide these records.

• A completed Certificate of Immunization Status (CIS) signed by a health care provider. Find the CIS form by visiting https://www.doh.wa.gov/SCCI and clicking on “Certificate of Immunization Status.”
• A CIS filled out by you or another parent/guardian with medical records attached.
• A CIS printed by a health care provider or school from the Washington State Immunization Information System.
• A CIS printed from MyIR which is a free Department of Health online tool that allows families to view and print their official immunization records themselves. Go to https://wa.myir.net/register to begin the sign-up process.

If you are requesting an exemption from one or more of the immunization requirements, you must provide the school a completed Certificate of Exemption. **A personal exemption may NOT be used for the MMR requirements**

Parent/guardian signature is only required on the CIS for students enrolling in conditional status.

• To enter school or childcare in conditional status, a child must have all the vaccine doses they are eligible to receive by their first day. Students in conditional status may remain in school while waiting for the next vaccine dose plus another 30 days' time to turn in medically verified record of vaccination

Vaccine Requirements For Kindergarten

• Five (5) Dtap, (4 doses only IF 4^th dose given on or after 4^th birthday).
• Two (2) Measles Mumps and Rubella (MMR) given after the 1st birthday and at least 28 days apart (A personal exemption may NOT be used for the MMR vaccine requirements).
• Four (4) Polio, (3 doses IF 3^rd dose is given on or after the 4^th birthday.
• Two (2) Varicella given on or after 1st birthday and at least 28 days apart OR physician's signed verification of disease from a physician OR positive blood test showing immunity.
• Three (3) Hepatitis B at required intervals

Immunizations can be obtained from the following places:

• Your family health care provider.
• Free child immunizations and low-cost adult immunizations are available at MultiCare Mobile Immunization Clinic.
